The Fairhaven community participation programme is a centre-based, holistic programme
focussed on community participation and integration for young people with disabilities,
thus increasing independence and active community involvement.
Individual programmes
will be implemented where individuals can choose and participate in real and meaningful
group activities. Activities available for service users will incorporate many learning
opportunities in both centre-based and community-based environments.
For example,
a holistic cooking programme would incorporate activities in both a community setting
(shopping) and a centre-based activity (cooking). Both programmes would include
training on personal safety and communication skills.
Other activities would be
accessing local library to enhance literacy skills, community integration by the
building of relationships with community members, friendships with other service
users, having fun, and becoming valued and active members of society.
There will
also be the provision for staff to undertake one-on-one activities with service
users, observation of skills and assessment of individual needs to assist in Individual
Planning.
All programmes will be developed from the Individual Planning process
based on Key Result Areas and the availability of activities and services available
in the local area for the young person to choose from.
Service delivery is focused
on specific outcomes for the person with a disability as identified in the Individual
Plan, which is a collaborative approach between Fairhaven staff, the young person
and their family/advocate.

This plan will be reviewed on a six monthly basis to
ensure it meets the changing needs of the young person, or to take advantage of
opportunities which may arise as a result of new or improved services becoming available
within the local community.
Partnerships will be forged with the local shire council
and others service providers through becoming a member of the Upper Hunter Rural
Web. We have already established links with service providers in the region including
the Mai Wel Group and Castle Personnel.
Transport options will be accessed through
Upper Hunter Community Care Inc and other local transport options. In addition,
once we reach our preferred maximum number of service users, a vehicle will be made
